These prices are effective as of April 15, 2005 but are subject to change without notice.

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
This list does not include prices for certain items that you may ask us to buy for you, such as cemetery, flowers, newspaper notices, clergy honorarium, certified copies, police escorts, etc. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected.
The general Price List represents the funeral services and merchadise of T. M. Ralph Funeral Homes and Trusted Funeral  Plans.

As a funeral directors, our calling imposes upon us special responsibilities to those we serve and to the public at large. An important obligation is to provide information so that everyone can make knowledgeable decisions about funerals and funeral directors.

In acceptance of our responsibilities, and as a condition of our membership in National Selected Morticians, we affirm the following standards of good funeral practice and hereby pledge:

  • To provide the public with information about funerals, including prices, and about the functions, services and responsibilities of funeral directors.
  • To afford a continuing opportunity to all persons to discuss or arrange funerals in advance.
  • To make funerals available in as wide a range of price categories is necessary to meet the needs of all segments of the community, and affirmatively to extend to everyone the right of inspecting and freely considering all of them.

PROFESSIONAL SERVICE
BASIC SERVICES OF FUNERAL DIRECTORS AND STAFF

Our fee for the services funeral director and staff, includes but is not limited to, staff to respond to initial request for service; arrangement conference with family or responsible party; arrangement of funeral; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notices; staff assistance prior to, during and following the funeral, including coordinating with those providing other portions of the funeral, e.g. clergy, cemetery, crematory, news notices, and other services related to the funeral. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, direct burials and forwarding or receiving remains.) ........................ $1625.00

LIMITED SERVICES

Limited services of funeral director and staff in a consultant capacity, arrangement of services and coordination, when burial or cremation is performed by another facility ........................625.00

EMBALMING

Except in certain special cases, embalming is not required by law. Embalming may be necessary however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial....………………...$ 345.00

CREMATION MERCHANDISE

If you want to arrange a direct cremation, you can use an alternative
container. Alternative containers encase the body and can be made of
materials like fiberboard or composition materials (with or without an
outside coverings), The containers we provide are designed for cremation
and include cardboard, fiberboard, veneer, and finished wood crematable
containers. All of our containers are 97% to 98% combustible and
contain less than 5% chlorinated plastics as required by Florida
Department of Environmental Protection.

 

Cremation is an irreversible process, therefore it is the

policy of the T. M. Ralph Family's funeral directors to

require identification prior to the cremation process.

In Addition it is the policy of this firm, to require

refrigeration, when embalming is declined.

 


Cremation Requirements and Disclosures

  1. A Cremation Authorization, must be signed by the legal next of Kin (s), and be in the possession of the funeral home.
  2. The Death certificate must be completed by the funeral home, from the information provided by the next of kin. It is then taken to the individual physician, or medical examiner of the district, who then must complete the medical portion of the certificate. The time frame for this is not always certain, since the physician is not always present in his office and weekends and holidays are involved.
  3. The cremation CANNOT be started until after 48 hours from the time of death. (Florida Statute 872.03)
  4. No human remains may be cremated until the medical examiner of the District in which the death occurred has reviewed the completed death certificate, and a permit has been issued by the State of Florida. The fee for this approval will vary depending on the district involved.
  5. The cremation schedule of the crematory will determine the completion and the return of the cremated remains.
  6. We cannot make any guarantees as to the delivery of the cremated remains because of the above variables. We recommend that you do not schedule any disposition of the cremated remains until the completion time of the cremation process can be determined.
  7. I/We acknowledge and understand that the cremated remains will be returned from the crematory in a plastic bag encased in a cardboard box, and unlessI/We have purchased an urn in which to encase the cremated remains, they will be returned to me/us in the plastic bag encased in a cardboard box.

The cremation process will be completed within 48 hours,

after all the above requirements and authorizations have

been fulfilled, subject to weekends and holidays.


Value Service Package Selection
When a burial casket or cremation container is selected, a value Service Package is available from our firm.  They include items most often chosen by families needing funeral services.  By utilizing these packages, you will be afforded a savings over individually selected items  When a merchandise package is not selected, any item(s) may be selected from our general price list. 
Our professional service fee is a non-declinable item.

(TO FREEZE THE COST OF A FUNERAL)

More and more concerned and thoughtful people are coming to us to arrange for their own funeral expenses in advance of need. We believe they are wise in doing so. If you have the forethought to plan ahead, you can protect your family from:

  2. The "Pre-Need Trust Plan"  GUARANTEES the cost of services at today's prices. You can pay for your plan in convenient installments if you wish, with no finance charges.
  3. The Plan is not insurance. There are no medical examinations necessary to enroll.
  4. All the money paid is placed in trust in a well-known Florida bank.
  5. There is no limit to the number of family members who can join the Plan. No membership fee is required.
  6. Our "Pre-Need Trust Plan" is regulated by the Florida Department of Banking and Finance, Tallahassee, Florida, in accordance with Florida Statutes, Chapter 455.
